• Hypertension patients
• Heart disease patients
• Kidney disease patients
• Elderly individuals
Support Includes:
Monthly medicine kits, laboratory testing support, prescribed medicines and health monitoring.
Beneficiary Selection:
Medical records, prescriptions, income verification and SDC assessment.
Annual Target: 50 beneficiaries.
Financial Commitment: INR 2,000/month, INR 24,000/year per beneficiary. Annual Budget INR
12,00,000.
